Students, please be prepared with the following information before you complete this RSVP form:
Your personal information (degree program, student ID number, UAZ & personal email address)
A photo & thank you message for your graduation slide (optional)
Any accommodations you or your guests may need for the event (e.g., ASL interpreting)
How many tickets you will require for your guests (up to 4)
The name of the faculty member who will hood you
The name of your thesis or dissertation (optional)
RSVP due by Sunday, May 3, 2026 at 11:59 p.m. (Arizona time).
