University of Arizona Required Forms

Work with your department's business office to submit the following documents to the Office of Sponsored Projects (520-626-6000, sponsor@email.arizona.edu): 

  1. UA Proposal Routing Sheet 
    F&A rate is 17.5%. In the box for justification of the reduced rate, write in the funding agency's cooperative agreement number, found on page 1 of the Cooperative and Joint Venture Agreement.
  2. Project abstract 
  3. Project budget 
  4. A copy of p.17 of the Cooperative and Joint Venture Agreement
  5. Completed SF-424 forms. 

See the Sponsored Projects Quick Reference for help with the SF-424. (For agencies that require 424 forms (especially NPS), send the completed forms to Sponsored Projects along with the 4 items listed above, where they will be signed, and then sent back to the NPS Research Coordinator).
